Patient Pre-Admission Process
Texas Health Orthopedic Heritage offers patients the convenience and privacy of an online registration process. Patients do not have to create an account; you will receive your forms via text and/or email. In these forms, you will be asked about your health history, medications and previous surgeries.
If you are a returning patient, you may not have to fill in all the information again but confirm that nothing has changed. You will have the option to update your information if needed.
It’s important to complete or update your online registration as soon as possible so that your medical team has time to review your information prior to your visit. We will call you if we have any questions or concerns. We look forward to seeing you!
Instructions
With our online pre-admission process, you will not need to create an account. You will receive either a text message or text message + email prior to your scheduled visit. The text message/email will include a link to access your forms
- Once you receive the link to your forms. Click on the link
- If you receive the link on your phone or tablet, you can fill out the forms directly from your phone/tablet. You will need to have access to the internet to complete the forms.
- If you receive the link via email on your computer, you can fill out the forms from your computer. You will need to have access to the internet to complete the forms.
- Fill out each question on the forms
- At the end of each form, click “Submit”.
If you have not completed a required question, the system will prompt you to go back and complete the required question(s). Your form cannot be submitted until all required questions are complete. If you are not sure whether you completed your form(s), please call us at 682-312-6464. Thank you!
